Introducing the MEDTRAC™ Health Advisor Questionnaires
As part of the ongoing development of the MEDTRAC™ Health Advisor, we are now making a series of structured clinical questionnaires available to both patients and our network of clinicians and therapists.
These forms enable us to collect more detailed, targeted health information on areas such as pain, metabolic function, inflammatory patterns, and neurological screening. They are currently delivered via secure Google Forms while we complete full integration into the automated MEDTRAC™ Health Intelligence system.
This is a temporary but strategic phase.
It allows us to:
- Test and refine question quality with early clients
- Confirm that the data collected is clinically useful
- Validate scoring logic and routing pathways
- Improve clarity before full automation
- Use the questionnaires immediately in Atlantis Clinic consultations
How It Works
These additional questionnaires are designed to be completed after our main General Health Intake Form, as they expand on specific areas identified during initial screening.
At present, submissions do not automatically notify the clinic. After completing a questionnaire, please send us a WhatsApp message so we can confirm receipt and attach it to your clinical record.
